The site also includes a health library, a weight loss tool kit, fitness center discounts at clubs in your area, health and wellness articles, newsletters and health calculators. Contact your employer for your company wide user ID and password.
	Healics Clinic Location List HRA participants unable to attend the work-site screening can view Healics contracted clinic locations prior to ordering an assessment kit.
	Ordering Your In-Clinic Assessment Kit HRA participants unable to attend the work-site screening can order a kit to be sent to their home. Participants must get authorization and an access code from their employer to use this option. Use the Healics Clinic Location List to locate a clinic prior to ordering.
	View Your Health Risk Report HRA participants can access their personal Health Risk Report online (to view updates, print additional copies, etc.). This option is only available to participants after they have received their original Health Risk Report in the mail - information from the original Health Risk Report is needed to create an online account.
	Schedule Your Worksite HRA Appointment Login here to view available appointment times for your worksite HRA and to schedule your appointment. A confirmation will be sent to the email address you provide.
